Sex need not be a part of breathplay, though it is for a lot of couples. Denial of air during sex heightens the intensity by inducing a hypoxic (oxygen-starved) state at the moment of orgasm. Of course this can lead to disastrous results if one isn't extremely careful.

I use it to instantly reaffirm my control over coffee. It can be considered pleasure or punishment depending on how and when it is administered. (I don't think coffee could ever see it as punishment.)

I don't advise using any other device to restrict airflow other than hands. Simply because it takes no time to instantly stop the play. Be aware that compression of the carotid arteries- (the arteries that supply the brain with oxygen) is involved here. This can cause fainting, stroke or even death. Persons with a history of stroke, high cholesterol or plaque developement should abstain from this practice.
